Who are we?
The Adelaide Primary Health Network (APHN) and Country SA Primary Health Network (CSAPHN) have jointly implemented the SA PHN Immunisation Hub which provides a range of approaches to support immunisation service providers and community including education, engagement, advocacy and resources. The activities of the Hub are aimed at increasing vaccination awareness, assisting the community to access immunisation information and service providers, increasing vaccine uptake and promoting the need for a well immunised community.
Immunisation Hub Coordinator
Angela is responsible for the development and ongoing coordination of the SA PHN Immunisation Hub. With over 35 years’ experience as a Registered Nurse, she has specialised in the field of immunisation for the past 17 years. Experience as the Local Immunisation Coordinator at the Adelaide Northern Division of General Practice, a Nurse Consultant in the SA Health Immunisation Section, an Immunisation Education Consultant and Immunisation Coordinator at Central Adelaide and Hills Medicare Local, Angela has a comprehensive understanding of immunisation program support required by providers and community.
Angela has contributed to the development of immunisation resources to assist providers with challenging aspects of the immunisation program including the comprehensive “What you need to know before you consent” resource folder and the PneumoSmart Vaccination Tool. Angela is passionate about the benefits of immunisation on public health and provides immunisation support and education to a wide range of health professionals across South Australia.
Angela has a Graduate Certificate in Nursing (Nurse Education), Professional Certificate in Immunisation, Certificate IV Training & Assessment and Certificate IV Frontline Management
Immunisation Hub Project Officer
Since graduating with a Bachelor of Nursing in 2006, Alex has followed her passion in primary health care. After working as a Registered Nurse in General Practice for 5 years delivering infant, adolescent and adult immunisation programs, Alex provided support to General Practice, delivered education to Practice Nurses and worked extensively across many projects at the Western Division of General Practice.
With this experience Alex worked as a project and care coordinator within the Closing the Gap, Aboriginal chronic disease program.
As the Immunisation Project Officer at the Adelaide PHN since March 2017, she has a primary focus on Australian Immunisation Register data analysis to ensure immunisation records of children accurately reflect their immunisation status. Alex provides project support to the Immunisation Hub, support and education to immunisation providers and represents the Immunisation Hub on various working groups.
Tracy works as the Nurse Consultant within the Regional Strategies and Service Design Team of the Country SA Primary Health Network.As a Registered Nurse she has over 25 years’ of health care experience.
She has qualifications in Diabetes Education and Management, Aged Care, Population Health, Training and Assessment, and Business Management (Human Resources).
She has managed and delivered numerous population health programs including diabetes and lifestyle management, health promotion, aged care and immunisation.
Tracy is passionate about immunisation, instigating successful initiatives including those to increase childhood immunisation rates in the rural sector.
